Confidentiality Policy

 

 

In order to protect the confidentiality of applicants and certificants of the certification program written authorization by the applicant or certificant is required before information is released regarding individual scores and status in the certification program.

 

(a). Disclosure of Examination Scores

 

(1). A signed authorization by the applicant or certificant granting permission to disclose individual certification scores will be accepted via mail, facsimile or courier service if the signature matches the application signature. The authorization must include the name and address of the party to whom the information is to be forwarded. Verbal authorization is not accepted.

 

(2). Disclosure of aggregate scores will be made to the public through written reports and to Boards of Nursing and Schools of Nursing upon written request. Disclosure will not take place that violate the certificants rights of privacy.

 

(b). Confirmation of Status

 

(1). It will be the responsibility of the applicant or certificant to provide the American

Academy of Nurse Practitioners Certification Program with required forms from state regulatory agencies, employers of others to whom disclosure of information is requested.

 

(2). A signed authorization by the candidate or certificant granting permission to disclose certification status will be requested as in part (a)(1) above.
