The information from this form will be reported to the Centers for Medicare and Medicaid Services (CMS), and will be used to refer you to a Practice Transformation Network (PTN). For more information on PTNs, please see "Next Steps" page.

Within a week of completing the registration form, you will receive an email welcoming you to the TCPi. This email will contain additional information on the initiative.
