AANP and Patient Priorities Care Launch Training to Advance Age-Friendly, Patient-Centered Care

The American Association of Nurse Practitioners® (AANP) is proud to announce a new collaboration with Patient Priorities Care (PPC) to develop an innovative learning opportunity: the PPC Microskills training program. This education equips health care professionals with practical, essential skills to deliver age-friendly, patient-centered care aligned with each individual’s personal goals, values and preferences.

Designed to be immediately applicable in clinical practice, the PPC Microskills curriculum introduces learners to the 4Ms of age-friendly care, beginning with the most foundational and transformative component — “What Matters Most” to patients. The training offers actionable approaches that can be seamlessly integrated into real-time patient interactions, supporting shared decision-making and improved care experiences, especially for individuals living with multiple chronic conditions.

A Practical Training Built for Real-World Care

The PPC Microskills training is divided into two key sections:

  1. Identifying Values, Goals and Priorities
  2. Applying Patient Priorities Care in Decision-Making

Learners can complete the training in the format that best fits their learning style. The program is available as either a video series or a set of print monographs, ensuring flexibility and accessibility across learning preferences.

Supporting Care for Patients With Multiple Chronic Conditions

Mary Tinetti, MD — Gladys Phillips Crofoot professor of medicine (geriatrics) and professor in the Institution for Social and Policy Studies, Yale School of Medicine — highlighted the importance of these microskills for today’s complex care needs: “It was a pleasure to work with our interprofessional team to create the learnings for patient priorities aligned care. Members of AANP provide much of the primary and specialty care for persons with multiple chronic conditions — the major users of health care. The number of people who have already accessed and completed the modules is very promising and heartening. We hope these modules help learners at all levels to feel better prepared to care for individuals with multiple chronic conditions.”

Nettles, DNP, MS, ACNP-BC, AOCNP, CNE, — assistant professor, Johns Hopkins School of Nursing — described how the partnership evolved into an effort with far-reaching meaning: “This was a such a rewarding collaboration that began as an opportunity to develop new educational content, but it quickly became so much more. Although we work in a variety of different health care settings, we realized we were encountering many of the same challenges in our roles as both clinicians and educators. A synergistic approach allowed us to actively contribute to a shared vision for the content and delivery of a comprehensive learning experience. Our work on the PPC Microskills highlights our commitment to the delivery of holistic care and thoughtful engagement with our patients.”
