This informative handout for the NP has been developed to facilitate conversations about human papillomavirus (HPV) vaccination with patients and their families. It provides an overview of the types of cancers that HPV can cause and discusses the rationale for why the vaccination should be administered during childhood for optimal protection. This handout displays the HPV vaccination schedule and also addresses some common questions and concerns that families may have about HPV vaccination.
Every person living with diabetes and also at risk of hypoglycemia needs to have a plan in place to manage the condition when it occurs. This practical and informational handout will facilitate conversations about hypoglycemia planning between the NP and the patients with diabetes for whom they provide care. The checklist format enables patients to easily keep track of things like the expiration date of and where they store their glucagon; the availability of fast-acting carbohydrate treatments, etc.; and the ability to share this information with their friends and family.
