AANP Tools and Resources
My Diabetes Plan - Available in English and Spanish, this is a provider tool that may be used to engage patients in the shared decision-making process when developing or changing their diabetes management plan. The tool was developed with the busy provider in mind and to help the patient better understand their treatment plan as well as the benefits and consequences of their choices. The Diabetes Medication - Available in English and Spanish, this chart is an education/resource tool developed with the patient in mind. This useful tool lists key components of oral and injectable antiglycemics on one side and insulins on the other side. Patients can use this simple and effective tool to help them understand basic information necessary to make informed decisions regarding therapeutic options. Patient Engagement Resources
In an ongoing effort to provide AANP members with effective tools to improve clinical practice, AANP, in collaboration with educational partner Med-IQ, has developed a series of educational resources that can be used during discussions with patients who have type 2 diabetes mellitus (T2DM). These were developed and produced through the support of an educational grant from NovoNordisk Inc.
The resources available are:
- 4”X9” single-sided handout that prompts patients to ask questions about their current T2DM management. The handout may be handed to patients or made available in the examination room
- Double-sided handout may be used to easily illustrate complex concepts to patients about appropriate T2DM management and GLP-1 receptor agonist use

Living with Type 2 Diabetes is one of AANP’s popular flipcharts created by expert NPs to support education of patients with Type 2 Diabetes. The flipchart comes with a 2-sided document of information to give to the patients to take home. AANP developed and produced the educational tool through support from Boehringer Ingelheim Pharmaceuticals, Inc, and Eli Lilly and Company.
